- The application for a passport document includes:
- data of the person to whom the passport document is to be issued: surname, first name(s), date of birth, place of birth, citizenship, gender, PESEL number;
- a reproduction of the handwritten signature of the person to whom the passport document is to be issued, except for a person who has not reached the age of 12 or who is unable to sign, or information about the absence of a signature;
- the names, surnames, types, series and numbers of identity documents held, PESEL numbers, if assigned, and signatures of the mother or father, legal guardians or curator present at the time of application, consenting to the issuance of a passport document to a person who lacks legal capacity or has limited legal capacity;
- correspondence address of the applicant;
- a statement on the veracity of the data provided, and a clause that reads: “I am aware of the criminal liability for making a false statement”;
- signature of the applicant;
- official annotations.
- The applicant may provide a cell phone number or e-mail address for transmission to the Register of Contact Information of Individuals.
- The statement of the truthfulness of the data provided shall be made under penalty of criminal liability for making a false statement, and the clause contained therein shall replace the instruction of the authority authorized to receive the statement on criminal liability for making a false statement.
