Registration of residence

You submit an application for registration of residence in person, no later than the next day after the expiration of 3 months from the date of entry into the territory of the Republic of Poland.

The requirement to submit the application in person does not apply to a minor.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions. You can also download the application form at any of our facilities,
  • a photocopy of a valid travel document or other document confirming your identity and citizenship (original for inspection), if you are unable to present a travel document or other document confirming your identity and citizenship, you may present evidence other than an official document that will allow you to confirm your identity and citizenship in a clear manner.
  • 4 photographs

If you have taken a job:

  • a written statement from the employer or entity authorized to entrust an EU citizen with work of the intention to entrust him with work or a certificate of work performance or an employment contract, contract of mandate;

If you are self-employed:

  • a written statement regarding entry in the National Court Register or the Central Registration and Information on Business Activity or other proof that the EU citizen is self-employed in the territory of the Republic of Poland;

If you are studying or undergoing vocational training:

  • a certificate from a state or non-state higher education institution of enrollment or referral to vocational training,
  • a document confirming that you have health insurance
  • a written statement that you have sufficient financial resources to support yourself and family members

If you are the spouse of a Polish citizen:

  • a document confirming your marriage to a Polish citizen (a current copy of the marriage certificate issued no later than 3 months before the date of application),
  • a photocopy of the spouse’s identity document (ID card or passport) – the original must be presented for inspection or its notarization attached;

In other cases:

  • document confirming possession of health insurance
  • proof of sufficient financial resources to support yourself and family members. Remember that the certificate of possession of means of payment in a bank or other financial institution, should be issued at least one month before the application for registration of residence.

Certificate of registration of residence of an EU citizen

An EU citizen whose residence has been registered is issued a certificate of residence registration by the governor.

Right of permanent residence of an EU citizen

You apply for a document confirming the right of permanent residence of an EU citizen in person, after 5 years of continuous residence on the territory of the Republic of Poland. An EU citizen who is the spouse of a citizen of the Republic of Poland acquires the right of permanent residence after 3 years of continuous residence on the territory of the Republic of Poland.

The requirement to submit the application in person does not apply to a minor.

In a particularly justified, motivated case, including due to the state of your health, the obligation to appear in person may be waived.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions. You can also download the application form at any of our facilities,
  • a photocopy of a valid travel document or other document proving your identity and citizenship (original for inspection), if you are unable to present a travel document or other document proving your identity and citizenship, you may present evidence other than an official document to prove your identity and citizenship beyond any doubt,
  • 5 photographs,
  • documents confirming 5 years of uninterrupted stay in the territory of the Republic of Poland,
  • documents stating the reasons for breaks in your stay, if any.

Document confirming the right of permanent residence The document confirming the right of permanent residence is valid for 10 years.

Registration of residence of a family member of an EU citizen who is an EU citizen

You submit the application for registration of residence in person, no later than the next day after the expiration of 3 months from the date of entry into the territory of the Republic of Poland.

The requirement to submit the application in person does not apply to a minor.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions (downloadable application).
  • a photocopy of a valid travel document or other document confirming your identity and citizenship (original for inspection), if you are unable to present a travel document or other document confirming your identity and citizenship, you may present evidence other than an official document that will allow you to confirm your identity and citizenship beyond any doubt,
  • a copy of the certificate of registration of residence of a citizen of the European Union to which you attach,
  • spouse – document confirming marriage (e.g. marriage certificate),
  • descendant (child or child of the spouse, up to 21 years of age or dependent of the EU citizen or his spouse) – a document confirming the existence of family ties and a document confirming age or certifying dependency of the EU citizen or his spouse (e.g. birth certificate),
  • ascendant (parent or parent of his/her spouse, dependent of the EU citizen or his/her spouse) – a document confirming the existence of family ties and a document certifying dependency of the EU citizen or his/her spouse.

Document confirming the right of permanent residence of a family member of an EU citizen who is an EU citizen

You apply for a document confirming the right of permanent residence of an EU citizen in person, after 5 years of continuous residence on the territory of the Republic of Poland.

The requirement to submit the application in person does not apply to a minor.

  • a photocopy of a valid travel document or other document confirming your identity and citizenship (original for inspection), if you are unable to present a travel document or other document confirming your identity and citizenship, you may present evidence other than an official document that will allow you to confirm your identity and citizenship beyond any doubt,
  • 5 photographs,
  • documents confirming 5 years of uninterrupted stay in the territory of the Republic of Poland,
  • documents stating the reasons for interruptions in your stay, if any.
Registration of residence of a family member of a non-EU citizen

Residence card of a family member of an EU citizen

You apply for a residence card in person, no later than the next day after the expiration of 3 months from the date of entry into the territory of the Republic of Poland.

The requirement to submit the application in person does not apply to a minor who has not reached the age of 6 by the date of application.

In a particularly justified, motivated case, including due to the state of your health, the obligation to appear in person may be waived.

Fingerprints are taken from a person who has reached the age of 6 by the date of application.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions (downloadable application).
  • a photocopy of a valid travel document or other document proving your identity and citizenship (original for inspection), if you are unable to present a travel document or other document proving your identity and citizenship, you may present evidence other than an official document to prove your identity and citizenship beyond any doubt.
  • 4 photographs
  • spouse – document confirming marriage (e.g. marriage certificate),
  • descendant – a document confirming the existence of family ties and a document confirming age or certifying dependency of the EU citizen or his spouse (e.g. birth certificate),
  • ascendant – a document confirming the existence of family ties and a document certifying dependency of the EU citizen or his spouse.
  • parents of a minor EU citizen – a document confirming actual custody of a minor EU citizen and a document certifying dependency of a minor EU citizen.
  • a copy of the certificate of registration of residence of the EU citizen, issued to the EU citizen with whom you reside in the territory of the Republic of Poland
Permanent residence card of a family member of an EU citizen who is not an EU citizen

You apply for a permanent residence card of a family member of an EU citizen in person, during the period of validity of the residence card of a family member of an EU citizen, after 5 years of continuous residence in the territory of the Republic of Poland,

The requirement to submit the application in person does not apply to a minor.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions.
  • a photocopy of a valid travel document or other document confirming your identity and citizenship (original for inspection), if you are unable to present a travel document or other document confirming your identity and citizenship, you may present evidence other than an official document that will allow you to confirm your identity and citizenship beyond any doubt.
  • 4 photographs
  • documents confirming 5 years of uninterrupted residence in the territory of the Republic of Poland
  • documents stating the reasons for interruptions in residence, if any

Exchange of certificate of registration of residence of a citizen of the European Union

You should apply for the exchange of the certificate of residence registration within 14 days after the prerequisites for the exchange of the document have occurred, i.e. if you have lost the certificate of residence registration in Poland, or if the data included in the certificate have changed, e.g. you have changed your name, the document has been damaged, or the image of your face has changed significantly.

If the certificate expires – apply for a new certificate at least 30 days before the expiration of the current certificate.

You submit the application in person. The obligation to appear in person does not apply to a minor EU citizen.

In a particularly justified, motivated case, including due to the state of your health, the obligation to appear in person may be waived.

Replacement of a document confirming the right of permanent residence

You must apply for replacement of the document confirming the right of permanent residence within 14 days after the occurrence of conditions for replacement of the document, i.e. when you have lost the document or the data contained in it have changed, for example, you have changed your name, the document has been damaged or the image of your face has changed significantly.

If your document has expired – apply for a new certificate at least 30 days before the expiration of your current document.

You submit the application in person. The obligation to appear in person does not apply to a minor EU citizen

In a particularly justified, motivated case, including due to the state of your health, the obligation to appear in person may be waived.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions (downloadable application). You can also download the application form at any of our facilities,
  • 2 photographs,
  • photocopy of a valid travel document or other document proving identity and citizenship (original for inspection), if you are unable to present a travel document or other document proving identity and citizenship, you may present evidence other than an official document that will allow you to confirm your identity and citizenship in an unambiguous manner,

Replacement of the residence card of a family member of an EU citizen

You must apply for replacement of the residence card within 14 days after the conditions for replacement of the document have occurred, i.e. if you have lost your residence card, the data in the card have changed, for example, you have changed your name, the document has been damaged or the image of your face has changed significantly.

If your residence card has expired – apply for a new certificate at least 30 days before the expiration of the existing card. You submit the application in person. The requirement to apply in person does not apply to a minor who has not reached the age of 6 by the date of application.

In a particularly justified, motivated case, including due to the state of your health, the obligation to appear in person may be waived.
