Replacement of the residence card

You replace the residence card if:

  • the data included in the current residence card changes, e.g. your name, address of registration;
  • your appearance changes to such an extent that the photograph placed in the existing residence card does not allow you to be identified;
  • you have lost your previous residence card;
  • your previous residence card has been damaged;
  • you have a permanent residence permit or a long-term resident of the EU and your card is no longer valid

You apply for replacement of your residence card in person, within 14 days from the occurrence of the above-mentioned circumstances. The application on behalf of a minor foreigner is submitted by the parents or court-appointed guardians, or one of the parents or one of the court-appointed guardians. When submitting the application of a minor who has reached the age of 6, his presence is required.

If you have an EU long-term resident permit or a permanent residence permit, you apply for a replacement card at least 30 days before the expiration of the residence card.

Documents needed:

  • application in 1 copy, completed in accordance with the instructions.
  • 2 photographs;
  • a photocopy of a valid travel document (original for inspection), in a particularly justified case, if you do not have a valid travel document and do not have the ability to obtain one, you may present another document to prove your identity;
  • documents confirming the circumstances indicated in the application:
    • a current copy of the civil status certificate in case of a change in the data included in the existing residence card (e.g. change of name) confirmation of permanent or temporary residence registration,
    • notification of the loss of the residence card issued by the police unit (optional)

You will also need confirmation that you have paid for the issuance of the document to start the proceedings.

How to apply

All applications can be submitted by mail to the address of the Lodz Voivodship Office in Lodz, 104 Piotrkowska St., submit at the application window No. 1 every day from Monday to Friday, 103 Piotrkowska St., from 8:00 am to 3:00 pm, except Tuesday from 10:00 am to 5:00 pm.
