Confirmation of possession or loss of Polish citizenship
To whom the application applies
- any person who intends to obtain official confirmation of possession or loss of Polish citizenship;
- an entity that demonstrates a legal interest in obtaining a decision;
- an entity that demonstrates an obligation to obtain a decision.
Necessary documents
The application for confirmation of possession or loss of Polish citizenship shall contain data of the person to whom the confirmation applies and his ascendants to the second degree, as well as information on relevant circumstances necessary to establish the factual and legal status. The required data include: name(s) and surname, family name, date and place of birth, father’s name, mother’s name, gender, citizenship, marital status, PESEL number, if assigned. The person and entity applying for confirmation of possession or loss of Polish citizenship is obliged to attach documents confirming the data and information contained in the application, unless obtaining such documents encounters insurmountable obstacles.
The minimum documents to be attached to the application are:
- a valid document confirming the identity of the person (Polish or foreign);
- a copy of the birth certificate;
- a document confirming marital status;
- a document confirming any change in personal information;
- documents proving that the applicant or his/her ascendants had Polish citizenship in the past (e.g. Polish passports, identity cards, military books), if the applicant has them in his/her possession;
- document confirming the acquisition of foreign citizenship.
Fees
According to the norms of the Stamp Duty Act of November 16, 2006, a stamp duty of PLN 58 should be paid. Payment should be made to the account of the City Council of Lodz, Finance Department, 5 Sienkiewicza St., Bank Pekao S.A. 50 1240 1037 1111 0011 0925 0073. Proof of payment should be sent to the Lodz Provincial Office in Lodz, 103 Piotrkowska St., 90-425 Lodz.

Receipt of the decision
Confirmation of possession or loss of Polish citizenship is issued by the competent voivode on the basis of art. 55 para. 1 of the Act of April 2, 2009 on Polish citizenship. It is an administrative decision that can be appealed to the Minister of Internal Affairs and Administration.
The decision can be sent by mail to the mailing address or collected in person at the headquarters of the Department of Civil Affairs and Foreigners.
