Pursuant to Article 217 § 2 of the Law of June 14, 1960, the Code of Administrative Procedure, the public administration body shall, at the request of the competent ecclesiastical authority, issue a certificate to ecclesiastical units on the possession of legal personality and on the bodies of legal entities. For this purpose, it is necessary to submit:

• an application for a certificate,
• proof of obtaining legal personality,
• the name of the body of the legal entity, the name of the person performing the function of the above-mentioned body,
• proof of payment of stamp duty.

Pursuant to Article 1(1)(1)(b) of the Stamp Duty Act of November 16, 2006, the issuance of a certificate upon application is subject to stamp duty. The stamp duty is PLN 17 for each copy of the certificate.
